By clicking "Agree" below, the member acknowledges that they understand and agree to all T's & C's contained within this document as legal and binding.

1. DURATION
1.1. This course runs though February 2016 at the notified time and dates.
1.2 All payments are non-refundable.
1.3 As this is a transformation course - you will need to attend ALL 8 classes please.
1.4 Regrettably if you miss classes, you will lose your spot in the program. No refunds will be offered in these cases, nor can the remaining classes be transferred to a 3rd party.

2. CONSENT & MEDICAL WAIVER

2.1 The member agrees to disclose any medical issues; pregnancy, etc. initially and to advise Sultry Studios timeously should they incur a recent medical problem during their contract.
2.2 The member understands that even full disclosure of medical history will not give them any rights to hold Sultry Studios, its employees and or members in relation to any medical issues which they may incur.
2.3 We record that our classes are physically demanding and do carry risk. The member warrants that they are physically and medically fit to attend classes and that they waive any claim against Sultry Studios, its members, employees, consultants and or members in relation to any loss, damage or bodily harm arising from their participation or attendance of classes on the premises of Sultry Studios.
2.3 The member further warrants that they shall not have any claim against Sultry Studios, its members, its employees and or members in respect of any loss, damage or harm sustained as a result of any reason including negligence in respect of your person or property while in on the premises.
2.4 Due to the competitive nature of the sport, as well as for your own safety, the member adheres to our Anti-Doping and Drug Free policies, which include your permission for random drug testing. Right of admission is reserved.

3. GENERAL
3.1 The member understands that instructors/trainers create class plans based on a large variety of factors to ensure a safe training environment and so will follow the activities given to them by the trainer.
3.2 The member agrees to adhere to the instruction of the trainer at all times for their safety and to create harmonious, fun classes for all involved.
3.5 The member consents to all photographs or live footage taken during class; rehearsals and/or performances be used by Sultry Studios for marketing purposes without additional consent unless specifically requested by the member.
3.6 The member agrees that they have read and understood the contents of this agreement and that they have entered this agreement voluntarily and without duress.
3.7 Terms and conditions are subject to change. Members will be updated timeously.
3.8 Our studio is non-discriminatory on race, gender, orientation or religious beliefs. Anyone not upholding these values will regrettably be asked to leave.

4. AGREEMENT
4.1. The member agrees to be bound to all the terms contained herein, has read and understood each point and shall perform in accordance with her contractual obligations.
