1.I understand that participation in classes does involve risk. I warrant that the child is physically and medically fit to attend classes and that I waive any claim against Sultry Studios, its members, employees and or students in relation to any loss, damage or bodily harm arising from their participation in classes or attendance on the premises of Sultry Studios.

2. Should they at any time feel dizzy or ill, they will advise the instructor and to refrain from further participation in the classes until they feel well enough and obtain their doctors advice.

3. I further warrant that I shall not have any claim against Sultry Studios, its members, it’s employees and or students in respect of any loss, damage, harm or death sustained as a result of any reason including negligence in respect of my person or property while in on the premises.

4. Due to the competitive nature of the sport, as well as for your my safety, I adhere to Sultry Studios' Anti-Doping and Drug Free policies, which includes my permission for random drug testing. R.O.A.R. No drugs, illicit substances or alcohol are allowed on the premises for any reason.

5. I hereby release and agree to hold Sultry Studios cc harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage, or loss to myself/the child and/or property that may be caused by any act, or failure to act of Sultry Studios, or that may otherwise arise in any way in connection with any services received from Sultry Studios cc. I understand that this release discharges Sultry Studios cc from any liability or claim that I, my heirs, or any personal representatives may have against Sultry Studios with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Sultry Studios cc. This liability waiver and release extends to Sultry Studios together with all owners, partners, and employees.

6. In line with the POPI act, you give consent to your / your childs' personal information required by Sultry Studios to process your the indemnity. This information is for the purpose of legal indemnification and will not be used for any other purposes other than processing your indemnity and correspondence.
